Welcome to the office of Drs. Duff and Cox, Ltd. located in Richmond, Virginia.  We are pleased that you have selected us to provide dental care for you and your family.

We will need some information in order to expedite your first visit to our dental office.  Please read the directions below and fill out the forms to the best of your ability.

The two forms that we have made available here are for your convenience so that you can fill them out in the comfort of your home.  This will help eliminate any waiting time at the office. Both of the following forms are Interactive PDF forms. All you need is Adobe Acrobat Reader (a free download) and you can fill them out on your computer, save them and submit them via email to the Office Coordinator, or you can print them out and bring them in completed at the time of your visit.

We also offer electronic versions of the abovementioned forms that you can securely submit online.


Afford Your Care

Helping make your dental care affordable and comfortable is important, and we proudly offer a variety of options to fit your dental care into your life.

Additionally, we are happy to estimate fees and insurance reimbursement before beginning your recommended treatments, and we will gladly file insurance forms for you. Optimal, comprehensive dentistry is what we practice, and we do not base our treatment recommendations on what your insurance will or will not pay. Our recommendations are based on what is best for you. Any treatment you choose to receive that is not covered by your insurance is your financial responsibility at the time of treatment unless other arrangements have been made, If you have any questions, don’t hesitate to ask. We love finding solutions that allow you to achieve the smile of your dreams.

New Patient Experience

Your first visit to Drs. Duff and Cox is an important time to get to know us and to being to understand all that we can accomplish to benefit your overall dental health. During this time, we will examine your entire mouth, take any necessary radiographs (X-rays), and work together to make a treatment plan that will move you toward keeping a beautiful smile for a lifetime.

Digital photographs of each tooth will be taken so that you can see exactly what your dentist sees. You will also have digital radiographs (X-rays) taken that will give you the highest quality images available with the least exposure possible. You can also bring to your appointment previously-taken bitewing X-rays (if less than a year old) and large panoramic X-rays (if less than three years old).

Following your evaluation and X-rays, your dentist will meet with you at this time to provide a professional cleaning and presentation of our findings regarding your oral health, including any recommended treatment. This is a perfect time for you to ask any questions and to learn all about your care – before we move ahead with your personalized treatment.
